Recognizing the importance of addressing social determinants of health and reducing health disparities within underserved communities, Legacy Health System has implemented this program. Community health workers are trained professionals who work closely with vulnerable populations to provide culturally appropriate healthcare education, advocacy support, access to resources like housing or food assistance programs, and facilitate communication between patients and healthcare providers.
